Revision rhinoplasty, sometimes referred to as secondary or tertiary rhinoplasty, is a surgical procedure performed to revise or repair the results of previous rhinoplasty surgery. Both aesthetic and functional problems of the nose may be addressed with revision rhinoplasty. Men and women who are physically healthy, realistic in their expectations and looking to improve the results of a previous rhinoplasty surgery may be good candidates for revision rhinoplasty. The procedure can take two or more hours to complete and may be performed in a surgeon's office-based facility, an outpatient surgery center or a hospital. Local anesthesia and sedation is often used, though general anesthesia may be used as well.

There are two options for the incisions for revision rhinoplasty. In what's known as 'closed rhinoplasty', the incisions are solely made on the inside of the nose. 'Open rhinoplasty', on the other hand, involves incisions made on the inside of the nose as well as a small incision made on the underside of the nose between the nostrils. Once the incisions are made, the skin is lifted to reveal the underlying bone and cartilage which are then sculpted to the desired shape. The skin is then replaced and stitched into place.

Compared to primary rhinoplasty procedures, revision rhinoplasty tends to involve less bruising, swelling, and pain. Splints and stitches are usually removed within six to seven days. While the swelling may take months to fully fade, bruising typically clears up within ten days. Patients often find that they can return to work within about a week after revision rhinoplasty, though more strenuous activities should be avoided for two to three weeks and risking injury to the nose and sunburn should be avoided for eight weeks. As with all types of surgery, there are potential risks associated with revision rhinoplasty and these may include adverse anesthesia reactions, infection, excessive scarring, prolonged swelling, small burst blood vessels, pulling of the nose to one side, and the need for further surgery.
